Filling out PDF forms

Many of the forms offered on this site are 'fillable' forms. Fillable forms are forms that you can fill out and print on your computer. We have also given you the ability to save these forms so you can store a copy on your computer or complete the form in more than one setting.

In order to use these forms, you will need to have the free Adobe Acrobat reader. For best results, version 7 or greater is recommended.

Download Adobe Acrobat Reader

You can download Acrobat Reader from Adobe's web page (www.adobe.com) or by clicking on the link above. Make sure to download version 7 or higher.

Open up the form you wish to fill out

Use the forms page to find the form you need. Clicking on the link to the right of the description will load the form in your browser.

Fill in the fields

Once the form is loaded, you will be able to fill out the form on your computer. At the top of the form, you should see something like this:

  • The Highlight Fields button will show you all the fillable fields on the form.
  • The Validate Form button will check to make sure you have filled in the most common fields
  • The Save button will allow you to save the form, with you data filled in. This will let you continue from where you left off at a later time.

The Print Form button will bring up the print dialog to allow you to print your filled out form

Print out the form, sign it, and send it in

Since most forms require a signature, you still have to mail in the form or bring it by our office. Once you print out the form, sign it, then mail it to:
